Social Work Supervisor - ElderPlace North Coast

Providence is calling a Social Work Supervisor (0.8 FTE, Days) to Providence ElderPlace North Coast Social Services in Seaside, OR.
Providence ElderPlace is an innovative program known as PACE (Program of All Inclusive Care for the Elderly), a comprehensive care model that offers health, housing, social service and care coordination solutions to their participants and peace of mind to their families. We have a total of 9 ElderPlace Health and Community Centers in the Region with a total of 8 in the Portland Metropolitan Area and one at Seaside, OR.
We are a looking for a Licensed Clinical Social Worker to become a member of our dynamic interdisciplinary team. In this position, the Social Worker will provide direct social work services to participants and their families as well as provide supervision to the Social Services Associate. This is an exempt, salary based pay position that will be scheduled 32 hours a week.
In this position, you will:
Provide oversight to social work for completion of comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of participants and their support systems.
Act as the geriatric/medical social work expert and clinical supervisor to social workers (Licensed Clinical Social Worker (LCSW), Clinical Social Work Association (CSWA), Social Service Administration (SSA) both within their immediate social work teams and ElderPlace Ministry.
Ensure that Social Work care planning is evidence-informed person-centered across the care continuum in collaboration with the Inter-Disciplinary Team (IDT).
Perform duties in a manner that respects participant self-determination, reduces health disparities/barriers to care, and adheres to the NASW code of ethics, Standards of Practice (SOP), and mission.
Required qualifications for this position include:
Current Oregon State Clinical Social Work license (LCSW)
State of Oregon approval to provide clinical supervision to LCSW candidates within 6 months of hire
Valid driver's license with dependable transportation
5 years of social work experience with the elderly
Preferred qualifications for this position include:
2 years of supervisory experience
2 years of experience working in a Program of All-inclusive Care for the Elderly (PACE)

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
